Fitting burn grab handles

Can anyone advise how to fit new grab handles to a medium burn? Do the foam packers have to be removed and if so how are they refitted in place?

The foam packers on the nut (inside bit) stay on, everything else goes on the outside, metal fitting (linked to grab handle, plastic washer then screw.
